1. Treat accountability as non-negotiable and always human

AI agents do not own outcomes. Humans do. This sounds obvious but in practice it gets eroded quickly -- especially when agents are performing well and the temptation is to reduce oversight and let them run.

The moment you cannot clearly name the human who is accountable for the output of an AI agent, you have a governance problem. Every agent in your operational stack should have a named human owner who is responsible for its performance, its errors, and the decisions it makes within its parameters.

This is not about blame. It is about clarity. Organisations that blur human accountability for AI outputs are creating risk that will eventually surface in a way that is very difficult to manage.

2. Define the decision boundary before deployment, not after

The most common agent governance failure is deploying an agent with an undefined or poorly defined decision boundary, the line between what the agent is authorised to do autonomously and what requires human review before action.

This boundary should be defined explicitly before an agent goes live, documented, and reviewed regularly as the agent's performance and your confidence in it develops. A useful framing is: what is the worst plausible thing this agent could do if it makes an error within its current parameters? If the answer is recoverable and low-stakes, the boundary can be wider. If the answer involves customer impact, legal exposure, or reputational risk, the boundary should be tighter and human review more frequent.

3. Build in review cadences, not just error alerts

Most agent oversight is reactive -- you get an alert when something goes wrong. This is necessary but not sufficient. Proactive review cadences, where a human periodically reviews a sample of agent outputs even when no errors have been flagged, are essential for two reasons.

First, agents can produce outputs that are technically correct within their parameters but wrong in context -- and that contextual wrongness will not trigger an error alert. Second, regular review is how you build the organisational knowledge to update and improve agent parameters over time.

4. Communicate with your human team about agents explicitly

One of the most underestimated leadership challenges of AI agent deployment is the impact on human team members. When an agent takes over tasks that a person was doing, or when an agent begins coordinating work alongside humans, the absence of clear communication from leadership creates anxiety, rumour, and disengagement.

They frame the conversation around what the human team members are now freed to do, not just what the agents are doing. 5. Treat agent performance management like team performance management

Agents have performance. It can be measured, reviewed, and improved. Leaders who treat agent performance management with the same rigour they bring to human performance management -- clear objectives, regular review, documented improvement actions, escalation protocols -- get significantly better outcomes than leaders who deploy and largely forget.

This also means being willing to decommission an agent that is not performing. The sunk cost of deployment is not a reason to continue with an agent that is producing substandard outputs or creating more problems than it solves.
